Once your class has been assigned with a date and time please print, fill out and mail this form with your registration fee to the address on top of the form. I must receive this prior to the start of lessons.

Spring/Summer Jump Start Program:
The registration fee is $30.00 per child. You will then owe the balance on the 1st day of swim lessons.
4 day Program offered Monday-Thursday mornings for a total cost of $80.00 per child. ($50.00 balance)
3 day Program offered Monday-Wednesday evenings for a total cost of $60.00 per child. ($30.00 balance)

Fall/Winter Jump Start Program
The registration fee is $30.00 per child. You will then owe the balance of $30.00 on the 1st day of swim lessons.
This is a 3 day Program offered Monday - Wednesday for a total cost of $60.00 per child.

*Two weeks back to back is recommended for all non-swimmers who have never had prior lessons, or who are timid and scared in and around water.

*It usually takes on average of 3-4 days for a non-swimmer to learn to swim unassisted in my program!

*After successfully completing a Jump Start Program and/or your child can swim 6-8 feet unassisted, your child will be able to join a Monthly Maintenance Program! The Monthly Maintenance Program is offered once a week on a continuous month to month basis.
